STUDENT/ATHLETE SPRING SPORTS SIGNUP FORM to signup for the sport(s) athletes want to try out for.
THIS IS REQUIRED FOR ALL STUDENTS INTERESTED IN PARTICIPATING IN SPRING SPORTS
Students/Athletes need only signup once on this form one time.
This is only a signup form. Athletes must attend at least one tryout in the sport they wish to participate in to be considered for and rostered on a team.
THIS FORM REQUIRED PRIOR TO TRYOUT/PRACTICES IN ALL SPORTS.
Athletes must submit this form prior participating in any tryouts or practice. Athletes that do not have this form submitted to the athletic department will not be allowed to participate in the tryouts until the form is turned in.
Athletes that participated in Fall Tryouts, Winter Tryouts or on Year-Round Sports have already submitted the Athletic Participation & Release Form and do NOT need to submit another form this school year.

TRYOUTS: INFORMATION
• There is no additional paperwork required prior to the tryouts (online sports registration & fees will be available for parents once the teams are posted, following the tryouts)
• Students are not required to attend all of the tryout dates to be considered for a team.
• The Athletic Department schedules multiple days for tryouts so that students can attend a tryout should there be a conflict on a particular day.
• Students are encouraged to attend as many tryouts as possible but must attend/participate in at least one tryout to be considered for a team.
• If a student misses a tryout date, he/she must notify the coach or Athletic Director (AD will forward to the coach). Athletes must notify coaches if they are not attending the final tryout.
• There are no make-up tryouts.
• Athletes wishing to tryout that have a Cast, Boot or other medical issue must provide a doctor’s note detailing what the athlete is cleared to do and what they are not allowed to do.
• Spring Sport Athletes are allowed to participate on ONE Spring season team only. Athletes interested in also participating on a Year-round team must consult with that coach, as well as the Spring sport coach to see if it is feasible to participate in a Spring Sport and Year-round team.
• Spring Sport Athletes can participate in C-Notes (choir) and must attend one of the two C-Note rehearsals weekly, no exceptions (C-Note rehearsals are Monday & Wednesday)
• Students trying out for more than one sport in the same season must let each coach know they are trying out for multiple sports and also indicate which sport they want to participate in prior to the final tryout for both sports.
Athletes will NOT be rostered on more than one team.
Athletes attempting to tryout for multiple sports will be placed on the first sport team that they may be rostered on.

TRYOUTS: SPRING TRYOUT / PRACTICE WEAR & EQUIPMENT
Student/Athletes should bring appropriate athletic clothing to wear at tryouts (no spaghetti-strap tanks or mid-drifts). PE clothes or athletic clothing should be worn to tryouts.
• Baseball/Softball – Glove; bat if you have one; cleats if you own. Bat requirements listed below.
• Field Hockey – Stick if you have one; cleats if you own
• Golf – Bring clubs to tryout & $10.00 for Driving Range Balls & Round of Golf (per tryout)
• Boys Lacrosse – Helmet, pads, stick if you own, cleats if you own – athletes cannot participate in some drills without proper safety equipment (Chaminade has limited equipment to borrow)
• Girls Lacrosse – Stick if you own, cleats if you own; Women’s Lacrosse Eye Guard/goggles for eye protection athletes cannot participate in some drills without proper safety equipment (Chaminade has limited equipment to borrow)
• Boys Tennis – Tennis racket
• Track & Field – Pair of running shoes, athletic clothing (spikes not required for first few weeks)
• Boys Volleyball – athletic clothing (knee pads optional)
• All Sports - Please bring water; can wear sweatshirt if temperature is cold
TEAM ROSTER POSTING – At the conclusion of a sport tryout, the team roster will be posted on the Chaminade Middle School Athletics Web-sit on the Middle School Athletics Team-Pages. Team rosters will be posted at approximately 8:00pm on the day following the last tryout for that sport. If roster does not appear, please check back the following day. Once teams have been selected, you will be given access to begin our online signup procedure.
ATHLETIC TEAM COMMITMENT & TEAM ROSTER WITHDRAWAL DEADLINE
Chaminade considers commitment an important trait that our students should consider prior to joining any team or group.
Athletes should not join a team if they cannot complete the season. Athletes that are selected and rostered on a team are expected to complete their commitment to the team for the entire season.
Students have ONE WEEK, after the posting of the team, to withdraw from a team without consequence by notifying the Athletic Director. After 1 week of the posting date, students quitting a team will be ineligible to participate on any Spring Season Sport Team, any Year-Round Sport Team and any Fall (2025) Season Sport at Chaminade Middle School.

TEAM REGISTRATION, HEALTHY ROSTER & ATHLETIC FEES
Following the posting of the teams, parents need to complete the registration process. Please go to your athlete's Team Page and click the links to
Healthy Roster and to
Formstack to pay Athletic Fees.
UNIFORM DISTRIBUTION
Team Uniforms will begin to be distributed the week of March 3. A schedule for distribution will be developed and posted. Uniforms will be issued directly to the each athlete at the gym on the team's scheduled date/time, during lunch. Athletes that miss that distribution may return the following morning between 8:00-8:45 am.
*Please note: Uniforms will
only be distributed to athletes whose Athletic Fees have been paid and Healthy Roster registration is complete. Athletes that have met these requirements, may return once completed.
ATHLETIC TEAM PRACTICES & GAMES – Regular Weekly Practices will be posted following the Spring Coaches Meeting (early February). Practice and games will be posted on the Chaminade Calendar in the Parent Portal (be sure to select the filter for Middle School Athletics. While on the Athletics Calendar, click on “Tryouts/Practices”, “Practices” or the event desired to see detailed information about practice or games. A game schedule/roster with venue directions will be issued to team members once all games have been confirmed and finalized.
